One Love is seeking a dedicated, compassionate, and experienced CBRS/Case Manager to support individuals with Intellectual and Developmental Disabilities (IDD) and other diverse needs. This role focuses on skill-building, supportive counseling, community integration, and coordinating services that promote independence and overall well-being. We are looking for someone who is patient, adaptable, and confident working with clients across a wide range of cognitive and functional abilities.

Position: Intellectual & Developmental Disabilities (IDD) CM/CBRS
Location: Boise Idaho

Responsibilities:
• Provide one-on-one community-based skill development services tailored to each client’s goals, abilities, and support needs.
• Assist clients in building skills related to daily living, communication, emotional regulation, social interactions, safety awareness, and community engagement.
• Develop and implement individualized service plans in alignment with Medicaid guidelines and agency standards.
• Deliver services in a variety of settings, including home, community, or office-based environments depending on client needs.
• Coordinate care with families, caregivers, case workers, support coordinators, and other community partners to ensure consistency and continuity.
• Support clients in accessing community resources, recreational programs, adaptive activities, and skill-building opportunities.
• Complete timely and accurate documentation, progress notes, and treatment updates in accordance with ethical and legal requirements.
• Participate in team meetings, staffing discussions, and training related to best practices in IDD support and community-based rehabilitation.

Qualifications:
• Bachelor’s degree in a human services–related field (psychology, social work, human development, education, etc.) required for CM roles; CBRS may require bachelor’s or enrollment in related educational pathways depending on Idaho Medicaid standards.
• Experience working with individuals diagnosed with Intellectual and Developmental Disabilities, Autism Spectrum Disorder, or related behavioral/cognitive needs.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to work independently in the community while maintaining professionalism, safety, and boundaries.
• Reliable transportation and the ability to travel within the service area.

Preferred Qualifications:
• Experience supporting clients in adaptive community programs, recreational skill-building, or structured developmental activities.
• Knowledge of Medicaid service models, documentation standards, and IDD-specific care planning.
• Familiarity with behavioral support strategies, trauma-informed practices, or adaptive communication methods.
• Experience with EHR systems and electronic documentation.
